Create a Food Allergy Card for School

A short communication card for caregivers, with optional guardian and contact fields. Use it alongside the school’s formal plans.

A practical starting point

  1. Follow the school’s own allergy procedures.
  2. Coordinate with your child’s healthcare team.
  3. Add a guardian name and a contact number if needed.
  4. Keep the school’s medical and emergency action plans separate and current.
Use School & caregivers Preset →

This preset exposes the guardian / emergency contact field and includes a caregiver checklist in the Detailed kit. It does not create medical instructions.
